Every person, firm or corporation desiring to establish, maintain or operate a food
products establishment shall make written application for a license therefor. The
application shall contain the name(s) and residence(s) of the applicant or applicants
depending upon whether the application is for an individual, for all members of an
association or partnership, or for the directors and officers of a corporation as
well as the location and description of the premises where the food products establishment
is to be operated. The application shall be filed with the health department.
