§ 15-6. Sliding fee scale for the child/adult dental health clinic.

  • (a)

    The name of this activity shall be referred to as the child/adult dental health clinic of the city health department, and it shall provide dental health care services to persons up to eighteen (18) years of age and older.

    (b)

    The director of the health department is hereby authorized to establish and implement the amended fee schedule(s) Children—Schedule A and Adults—Schedule A2 for dental health services provided in the child/adult dental health clinic of the health department [on file with the city].

    (c)

    The director of the health department is hereby authorized to charge for the fees specified in subsection (a) utilizing a sliding fee scale in accordance with the annually issued federal poverty income guidelines, and said income guideline changes may be adjusted as needed by the health department director so as to conform with changes in the federal income guidelines as they occur. A currently valid sliding fee scale is attached as exhibit 1, attachment 2 [on file with the city].

    (d)

    A clinic user fee of twenty dollars ($20.00) will be charged to all clients, under eighteen (18) years of age and who are at one hundred (100) percent of poverty level or below (zero (0) percent pay), and who are non-Medicaid eligible. A clinic user fee of twenty-five dollars ($25.00) will be charged to adults (persons eighteen (18) years of age and older) for initial oral examinations, and any needed dental services provided would be based on the amended Fee Schedule-A2. submitted by the department [on file with the city].

    (e)

    The health department shall make reasonable efforts to collect the fees provided for in subsection (a), but if administrative costs of recovering a fee should exceed the fee, then such fee may be waived; and provided, further, that the health department shall not deny such dental services necessary to alleviate pain because of a person's inability to pay for same.

    (f)

    Revenues collected from the delivery of these health services in the dental health clinic will be identified, reported and expended in accordance with the state department of health contract provisions.
