§ 2-326. Persons required to report; time to report; place to report.
(a)
A city official or employee who has knowledge of a violation of any of the provisions of this ethics code shall use his best effort to report this violation as provided within five (5) working days after the person has knowledge of a violation. A city official or employee shall not delegate to, or rely on, another person to make the report. Any city official or employee who has knowledge that a violation of the ethics code has been committed and intentionally fails to report such violation is subject to the penalties herein.
(b)
A report made under this section shall be made to the ethics compliance officer or his or her designee;
(c)
A report shall state:
(1)
The name of the city official or employee who believes that a violation of a provision of the ethics code has been or may have been committed;
(2)
The identity of the person(s) who allegedly committed the violation;
(3)
A statement of the facts on which the belief is made; and
(4)
Any other pertinent information concerning the alleged violation.
