§ 5.5-23. Attendant's report.

The attendant on each emergency run will make a report, using the ambulance patient record form to be provided by the fire department. The holder of the ambulance operator permit shall preserve the reports for two (2) years from the date of the emergency run and will submit to the fire department, on the first of each month, a copy of the report on each emergency run made in the preceding month by any ambulance being operated under authority of his permit.
